Working with local health boards, Telemedicine monitors ‘vital signs’ in people’s own homes. It often means an early discharge for patients, or can sometimes replace a hospital visit completely. Telemedicine is an ideal service for rural and remote areas where there is no hospital nearby.
How it works:
- A healthcare professional refers a patient to Hanover Telecare, usually via telephone or fax.
- Hanover Telecare creates a unique electronic patient record.
- The healthcare professional inputs all necessary clinical information, instructions and escalation process into the patient's monitoring plan via a web browser and an intranet connection.
- Hanover Telecare delivers and installs the S21 monitor and teaches the patient (and carer if necessary) how to use it.
- The patient then takes their own clinical readings after which they send the readings over a telephone line to Hanover Telecare monitoring centre where it is downloaded into their plan.
- The healthcare professional accesses the patient's readings when required via a remote PC.
- Hanover Telecare acts upon any clinical exceptions and alerts that have been gathered during the monitoring period and carries out a predefined escalation process set by the healthcare professional.
What does the S21 monitor measure?
- Blood pressure
- Pulse rate (3 lead ECG)
- Breathing rate and amplitude
- Blood oxygen saturation level
- Temperature
- Spirometry
- Body Weight
- Glucometry
The flexibility of the S21 monitor makes it ideally suited to a wide range of clinical applications for patients wherever they may be - at home, in residential or nursing homes, prisons or in a remote location.
Some applications include:
- The post acute care of patients with Chronic Heart Failure and COPD
- Management of chronic diseases in both secondary and primary care.
- The diagnosis and management of Hypertension.
- Screening for Ischaemic Heart Disease.
- Arrhythmia diagnosis.
- Screening and diagnosis of Obstructive Sleep Apnoea.
- Titration of therapeutic devices.
- Management of preclampsia
- Healthcare screening for workplace employees.
